If you know someone that is interested in receiving service and they mention your name at the time of our Assessment, we will send you a gift card for $100 in groceries if that person starts service with us.

Terms and Conditions of the referral fee are:

1. Services must be rendered in Toronto and Ottawa Area
2. All services rendered are subject to staff availability
3. This gift is valid only for new clients of Affinity Health Services
4. One referral gift per household only
