Companion Care
The goal of our Companion Care services is to cultivate a positive state of mental and emotional well-being for our clients. As part of our special in-home assessment, our Case Managers can select a companion for you or your loved one who is compatible with your personality preferences.
Personal Care
You can feel confident that our qualified and professional Personal Care workers are knowledgeable and proficient, and are always ready to assist you with tasks such as bathing, dressing, and anything else that you may need help with.
Transitional Care
Designed to help prepare patients discharged from the hospital to smoothly transition to the next appropriate destination. As needs change, our team works with patients and their families to update their care plans and services. Eligible patients and families from hospitals are referred to the program.
Home Support Services
Home Support Services at Affinity Health have been thoughtfully designed with you and your needs in mind. Suitable Home Support is an important factor in maintaining a safe, comfortable, and clean environment.
Hospital Discharge
Returning home after being in the hospital is always an exciting time, but it can also involve worries and questions about what life will be like once you’re home again. We will help you address your questions and concerns, by offering help and support for individuals and families through the transition back into life at home after leaving the hospital.
Nursing Care
Whether you’re at home, in a hospital, or in a care facility, Affinity Health can help with all your medical needs. If you require changes to your wound dressings, regular injections, or even a regular nurse visit to monitor your health and progress after surgery, our caring team of professional, caring Nurses will be there.
Meal Preparation
Through an intensive screening and matching process, we will find and designate a caregiver to you who is as capable and compassionate as you deserve, and will always treat you with the high level of kindness you expect.
Workplace Injuries
We can take on tasks such as ordering and collecting medications, setting up doctor’s appointments, managing at-home recovery, coordinating transportation and other relevant tasks. Our Case Managers in Toronto & Ottawa can also provide advice and support as you determine what services you will require upon returning home.
Motor Vehicle Accidents
Our Personal Support Workers (PSWs) are available to help navigate and deal with the difficulties that may come as a result of acquired brain injuries or other issues attributed to an accident.
Special Needs
Our goal is to help seniors Affinity Health for as long as they choose, and whether our client has Parkinson’s disease, Dementia or any other ailment that requires special attention, this goal of remaining at home is attainable.

Understanding Home Health Care
Whether you are senior dealing with age-related issues, or someone who has been in a severe accident or recovering from surgery, many of the services we provide are similar in nature.
